The notifications for this form submissions will be sent to cgoodson@blinkmarketing.com, bscarborough@blinkmarketing.com, and thyssong@blinkmarketing.com. Also, please note that we added an Email field since you require that the user submitting the form will also receive the confirmation email. Please note that the email subject cannot be customized as Wufoo will use the form's title as the email subject.
